Home Hemodialysis prescription is more than just adjusting treatments to achieve target Urea Kt/V.  Home HD prescription should include goals to achieve adequate small and middle solute clearances, optimum volume management and residual renal function.  The prescription must also be patient-centric considering timing of the therapy and logistical concerns that addresses maintaining a quality of life that minimizes major disruptions to the patient’s lifestyle.
